MeroMed Community Programs
HIV Prevention & Care Program:
Our HIV Prevention & Care Program offers a range of services to support individuals in managing their health, reducing stigma, and preventing the transmission of HIV. These services include:
• Free and confidential HIV screening
• Linkage to care and treatment services
• Pre- and post-exposure prophylaxis (PrEP/PEP) education and referrals
• Ongoing case management and medication support
Mental Health & Counseling Services:
Our Emotional Wellness Program provides individual and group counseling sessions, trauma-informed care for stress, anxiety, and depression, and crisis intervention support. Additionally, we offer referrals to long-term care providers. This program empowers our community to heal, build resilience, and thrive emotionally.
Chronic Illness Management Program:
We assist individuals living with chronic conditions such as diabetes, hypertension, heart disease, and asthma. Our services include:
• In-home health assessments
• Health coaching and care planning
• Medication education
• Ongoing check-ins and support
These services aim to reduce emergency visits and improve long-term health outcomes.
Community Education & Wellness Outreach:
We lead community education and wellness outreach initiatives, including health workshops in schools, churches, and local centers. Additionally, we offer nutrition and fitness classes and conduct wellness screenings (blood pressure, glucose, BMI) to promote healthy living.
Educational Materials in Multiple Languages.
Impact: Promoting prevention and empowering individuals with the knowledge to take control of their health.
Home-Based Care & Access Services:
For individuals with limited mobility or no transportation, these services provide:
• In-home visits by licensed providers
• Referrals for telehealth or transportation support
• Navigation assistance for insurance, social services, and local resources
